Social Services Verification

If you are receiving benefits from local, state and/or Federal Social Services you may be required to have a Social Services Verification form completed by our office.  This verification details the types and amounts of financial aid you are scheduled to receive.  Once submitted to our office, these completed requests will be mailed to the Social Services Office indicated on the form  within 72 hours unless you have indicated on the form your wish for it to be mailed or picked up by you.
