To apply for the program:

1. Complete an application (word doc) and preliminary assessment form (pdf).
   Students submitting both forms will schedule an interview with a Student Support Services Program staff member. During the interview, the Student Support Services Program staff member will explain various services available for students.
2. Student will then be notified if accepted into program.
