Registration and Records

Forms

Completed forms can be submitted to the Registration & Records Office at Records@aims.edu or in-person on any Aims campus unless noted below. All emailed forms must be sent from the student's Aims email address (@aims.edu).  

General Forms

  • Request to Late Drop or Withdraw from a Course

    If you were unable to drop or withdraw from your course by the indicated deadline and have extenuating circumstances that have caused you to miss that deadline, you may fill out the Administrative Registration Change Petition form to request a late drop or withdrawal from your class(es).

    *Note: this is not a tuition appeal and in the case of an approval of this form, it will not result in a tuition refund.

  • Request to Late Add a Course

    If you are attempting to add a course to your registration but the deadline to do so has passed, you may fill out the Course Late Add form to request to be added into that class(es).

    *Note: in order to be late added into a course, you must have been attending on or prior to the drop deadline for the course in question.

  • Change Your Contact Information

    If you want to update your legal name, preferred name, address, or advisor, you may do so by submitting the Information Change form. If you are a current student, you can update many of these items in your MyAims account as well.

  • Change Your Major

    If you would like to update or change your major or update your current catalog, you may do so by submitting a Major Change form.

    *Note: For questions regarding major selection, please contact your Pathway Advisor at askanadvisor@aims.edu or by calling (970) 339-6443.

  • Students Under the Age of 18

    If you are a student under the age of 18, you must complete the Parent/Legal Guardian Statement for Student Under the Age of 18 form.

    *Note: This form requires the signature of your parent or legal guardian in order to be approved.

  • Student Records Release

    In compliance with the Family Education Rights and Privacy Act (FERPA) regulation 34 C.F.R. part 99.30, the disclosure of information from a student's education record requires the parent or eligible student to provide signed and dated written consent before Aims Community College discloses personally identifiable information (PII) from the student's education records, except as provided in section 99.31.

  • UNC Co-op

    Full-time UNC students can qualify for one free course (up to five credit hours) per the Cooperative Agreement between Aims and UNC. If you are interested, please complete the Cooperative Registration Agreement with UNC form. Completed forms can be sent to cashier@aims.edu.

    *Note: Forms that are only partially completed will not be accepted.

  • CSU Co-op

    Full-time CSU students can qualify for one free course (up to five credit hours) per the Cooperative Agreement between Aims and CSU. If you are interested, please complete the Cooperative Registration Agreement with CSU form. Completed forms can be sent to cashier@aims.edu.

    *Note: Forms that are only partially completed will not be accepted.

  • Applying for Graduation

    In order to have a degree or certificate awarded, you must apply for graduation from that program. Any programs that have not been approved for graduation will NOT be posted to a student's official record. Contact GradApp@aims.edu for more questions.

    *Note: Please submit your application for graduation whether you plan on participating in the graduation ceremony or not.

  • Need a Fresh Start?

    Had a rough semester? You may attempt to improve your overall GPA by eliminating all previous coursework from your current GPA calculation by completing a Fresh Start Petition form.

    *Note: Only student returning to Aims after an absence of two calendar years or longer are eligible for this. Additional details about this process are located on the second page of the form.

  • Audit a Course

    If you would like to audit a course, please fill out the Audit Request form.

    *Note: Your instructor will need to sign the form before you submit.

  • Requesting Non-Disclosure of Directory Information

    If you would like to request that your directory information be restricted from public view, please complete the Request of Non-Disclosure of Directory Information Request form located here.

Residency Forms

  • Resolve Pending Residency Status

    If your residency status is currently listed as “pending,” it means that your residency could not be determined from the information listed on your most recent Application for Admission. To remedy that, you may fill out the Admissions Tuition Classification Addendum form for reclassification by the first day of the semester in which you intend to register for classes.
    *Note: If you fail to provide this Addendum form prior to the deadline, your "pending" residency status will be automatically changed to "Out-of-State" and you will be responsible for paying your student account based on the "Out-of-State" rate until your Colorado residency is determined.

  • Request In-State Residency

    If your residency is currently listed as Out-of-State for tuition classification purposes and believe you qualify for In-State classification, you may request a change by completing the Petition for In-State Tuition Classification form.

    *Note: This form must be notarized and requires additional documentation attached in order to be accepted for review. There are notaries available on the Greeley campus in the Welcome Center, if needed.

  • Request In-District Residency

    If your residency is currently listed as In-State for tuition classification purposes and believe you qualify for In-District classification, you may request a change by completing the Request for In-District Residency form.

    *Note: This form can be submitted together with the Petition for In-State Residency form, if desired.

  • ASSET & High School Graduation In-State Exception Request Form

    Under House Bill 22-1155, you have the option to petition for In-State residency under your high school graduation completion or GED earning, if you do not qualify under the standard tuition classification process.

    ASSET:
    - If you are a student with undocumented or DACA citizenship and you wish to petition for ASSET tuition rates and classification status, you will need to submit the ASSET & High School Graduation In-State Exception Request form located here along with the "COF Affidavit" form and a copy of your final high school transcript (or Colorado-based GED) to Records@aims.edu for consideration.

    - If you are wanting to petition for In-State tuition rates and classification status under the High School Graduation Exception, you will need to submit the ASSET & High School Graduation In-State Exception Request form located here along with a copy of your final high school transcript (or Colorado-based GED) to Records@aims.edu for consideration.

  • COF Affidavit

    If you are a student with undocumented or DACA citizenship and you wish to petition for ASSET tuition rates and classification status, you will need to submit the COF Affidavit form located here along with the "ASSET & High School Graduation In-State Exception Request" form for consideration.

    *ASSET qualification also requires proof of high school graduation.

  • Request In-State Residency under Historical Ties to Colorado

    If your residency is currently listed as Out-of-State for tuition classification purposes and you are have ties to an American Indian tribe(s) with historical ties to Colorado, please complete the American Indian Tribes with Historical Ties to Colorado In-State Exception Request form.

  • Request for W.U.E. Residency

    If your residency is currently listed as Out-of-State or Pending for tuition classification purposes and you are a resident of one of 16 eligible states/territories in the western United States, you are eligible to apply to participate in the Western Undergraduate Exchange (WUE) program. WUE students pay reduced tuition rates that are equal to one and one-half times the normal in-state rates instead of the out-of-state rates.

    *Note: W.U.E. states are currently Alaska, Arizona, California, Colorado, Commonwealth of the Northern Mariana Islands, Guam, Hawaii, Idaho, Montana, Nevada, New Mexico, North Dakota, Oregon, South Dakota, Utah, Washington, and Wyoming.

  • Active Duty In-State Exception Request Form

    If you are on active duty in the United States Military (minus Colorado National Guard) or a dependent of an active duty service member and wish to petition for In-State residency via an exception, please complete this form.

  • Request In-State Residency as a Veteran

    If your residency is currently listed as Out-of-State for tuition classification purposes and you are an Honorably Discharged Veteran of the Armed Forces of the United States (Army, Navy, Air Force, Marines, or Coast Guard), who is currently planning to make Colorado their permanent home, but does not yet qualify for In-State tuition as one-year residents of Colorado, please complete the US Armed Forces Veteran In-State Tuition Application form.

    *Note: This form does not extend to members of the Colorado National Guard.

  • Request In-State Residency under the GI Bill®

    If your residency is currently listed as Out-of-State for tuition classification purposes and you are veteran or dependent of veteran utilizing GI Bill® benefits at Aims Community College, please complete the GI Bill® Resident Rate - In-State Tuition Application form.
